Service Description

We will prepare both the Durable Power of Attorney, and the Health Care Surrogate documents, travel to grantors set destination to notarize the documents. A notary will travel to your location, review, sign and stamp your document for this fee if: 1. You're within a 15-mile radius of Notary. 2. Durable Power of Attorney documents require 2 witness signatures. You must provide your own witnesses. (The notary will not act as a witness) ***Note: Scheduled time slots for this service are not to exceed 30 mins. If Notary arrives on site and the agreed upon time slot surpasses 30 mins due to party being unprepared with required valid Identification and legible document, then a $25.00 fee will be added for additional time. *Must always have a valid identification that is not expired. **All parties must be at least 18 years of age. ***Competency of party presenting must be intact. If one or more of the party presenting is unable to understand document and provide clear acknowledgement of agreement the notary holds the right to refuse notarization of the document without refund.
